Our quality care team provides full range of primary and preventive health services using a Medical Home model. We take 'ownership' interest in our patients' long term health outcome and wellness.

Our medical providers are board certified in Family practice, Internal Medicine and Geriatrics (1992-2002). They represent over 75 yr of cumulative clinical patient evaluation and management experience. We have four office locations throughout Herkimer and Oneida Counties for our patients convenience: Little Falls, Mohawk, Dolgeville and Rome. Medical Home model of health care delivery used by our group is focused on patient centered, continuous, coordinated quality care that relies upon desired long term health outcome.

Our Medical Group History
Established in 1987 in Little Falls, NY by Dr. Deepak Buch, our practice has grown to cover communities in Herkimer, Oneida, Otsego, Montgomery and Fulton Counties. Our physicians have admitting privileges at St. Elizabeth Hospital in Utica and Little Falls Hospital in Little Falls, NY.
